About Us

CenterWell is a Senior focused primary care center designed for patients 65 and older. Our Care Team includes a doctor, nurse practitioner, medical assistants and a referral coordinator. Patients also have access to nurse care coach, behavioral health specialist, social worker and a clinical pharmacist. The whole team works together to meet all aspects of a patients health needs – physical, emotional and social. The idea of “care centered on you” is all about giving seniors the time, space and respect they deserve.
